B1512-
Air Flow Control Actuator (Position
Sensor) and/or Its Circuit Malfunction

B1513-
Temperature Control Actuator and/or
Its Circuit Malfunction

Is it very cold where you live right now? The actuators could be quite cold and too stiff to move in the cold properly (or in the time that the HVAC module thinks they should move). If it isn't cold the linkages and gears that make everything work might be hard to move from dirt and other crap getting stuck in them. You'd have to try and clean them up as best as possible.

I forget if the '04 have a pollen filter, but if that has never been changed it could be pushing through and not filtering a lot of dirt and getting everything gooped up. If it doesn't have a filter at all then even worse for dirt.

I'd start with whether it was cold or not when this happened. If not, something is jamming up the Air flow actuator and Temp control actuators - or maybe one or both of them just quite working.
