The route of the passages changed over the years and engine type.

For the most part, the exhaust side comes from rear cylinder (#4) through a passage at the rear of the head to the EGR valve on the intake manifold.
From the valve, the gasses are routed to some place (one or four places) on the intake. The route varies by year and engine type. The latest 16 valve motors have a single tube, external to the intake, that LOOKS like it would be MUCH easier to clean than the earlier 16v internal passages.
