Eight of the blocks (drilled) go in place with stock Body Mount areas. The extra four (not drilled) go with the stock body mount spacers:
"Next, as you read on the previous page, you were supposed to keep an eye on the body spacers as you were jacking up your truck. Place your un-drilled box tube body blocks in each of these spaces. Place the stock spacer on top of the block. Just set them in place for now. The compression of the body when lowered should hold them in place. For future reference, when you get the truck together, you may want to find a buddy with a welder, or a local exhaust shop to tack-weld each of these blocks in place."

The extra four are just spacers -they carry load, tranferring the weight of the body onto the frame. But these are not bolted in. You can glue them on with Goop automotive glue (worked for me).

and on your shifters yes to do need to mod them the transfer case shifter will foul up in 2wd with a 2 inch lift on the mount panel of your centre console, (mine is just removed temporarily) cant comment on gear shifter ATM cos currently mines an auto.
but on mods ive been considering whether i can cut the transfer case lever and tap a thread in each end install a section of rod in there and use some loctite and a couple of nuts to lock into position
mine came with my lift kit (which came with my vitara so i didnt actually buy it anywhere), but which was by a mob call DDS but i cant seem to find them online, but the phone number on the bumper sticker that came with it (which is all i can find) has a australian phone number (07)33252640 which i think is Queensland but thats a long way from me
