take a pry bar and pry it off. is the rotor so worn that it has a lip on it? if so i would get a new rotor and take a grinder to the one on there.

when removing the rotor, on the front, on the back side of the hub, where the caliper bolts to, there will be to 17mm bolts that hold the caliper bracket on, remove those, then remove the two phillips head screws that are holding the rotor to the hub, you do not have to reinstall these, but it a good idea too, so that way the rotor is held inplace while replacing the pads, and the same goes for the rear, with one exception, the rear rotor also house the emergency brake shoes, if they are adjusted to far out, the rotor/ inner drum will not comes off easliy, you will need to adjust the shoes to where there is free play, then it should jus come right off

Nice post. Done numerous pad changes before but this is the first time on my Reno. I went 92,000 miles before my first change. I am VERY HAPPY with that!
